Trusted Post-Acute Care Services You Can Count On

As you prepare for discharge from the hospital, our care-management teams can help guide you to the appropriate post-acute care. As part of our Post-Acute Care Collaborative (PACC), Keck Medicine of USC partners with select skilled-nursing facilities, home health agencies and hospice organizations to support you in your ongoing care.

Choosing a post-acute care provider can feel overwhelming. We vet our care partners closely and can help you decide which option is right for you. These home health agencies were chosen because they met certain quality and clinical criteria. By partnering with and holding select organizations in the community to a high standard, we help you feel comfortable in finding the highest-quality post-acute care. We continually collaborate with our care partners on shared goals related to improving quality, communication and patient experience.

Home Health and Home Infusion Services

Home health care provides skilled, short-term services in-home. These services are typically ordered by a doctor to help with recovery following an inpatient hospital stay. In general, the goal of home health care is to help you get better and regain your independence.

Home health services are provided by registered nurses, physical therapists, occupational therapists, home health aides and social workers — all of whom are focused on providing the right level of care that you need.

Skilled-Nursing Facilities

After being in the hospital for an accident, serious illness or surgery, patients may need additional medical, nursing and personal care. A skilled-nursing facility, or nursing home, houses doctors, nurses, rehabilitation specialists, and other medical professionals and activity programs — all to ensure you get the level of care you need.

People who qualify and meet the clinical criteria for 24-hour care in a nursing home can select either short-term or long-term care:

  • Short-term care for people who need rehabilitation after surgery, like physical therapy after a hip or knee replacement
  • Short-term care for people who need skilled nursing to recover from an illness
  • Long-term care for people who need help with activities of daily living, like bathing, dressing or eating

Hospice Care

Hospice care is for people in the final stages of life who want to focus on comfort as the top-most priority for their medical care. When transitioning to hospice care, there is a shift from life-prolonging treatments to supportive treatments.

To qualify for hospice services, your doctor will certify that a life expectancy is 6 months or less. It is important to choose a hospice agency that aligns with your care priorities and that will support your needs. It is your right to speak with multiple hospice agencies before you decide on one to enroll with. Upon enrolling, you sign a statement choosing hospice care instead of other insurance-covered treatments or visits for your life-limiting illness and related conditions.

You can receive hospice care services in your home or at a facility such as a nursing home. You can always sign off hospice services if the care approach is not right for you.

Home Health and Home Infusion Services

Providers participate in the Keck Medicine of USC Post-Acute Care Collaborative (PACC) on a voluntary basis with no financial incentives for either party. You are not required to select a provider from the Keck Medicine of USC PACC.

Centers for Medicare and Medicaid Services (CMS) quality ratings are part of our process in selecting the organizations on this list. When a PACC partner’s star rating changes, our team works closely with the partner to create a detailed performance improvement plan. You can find the most recent CMS quality data for all home health agencies online at: www.medicare.gov/care-compare.

Please be aware that some insurance companies have contracts with specific providers, suppliers and facilities, which may affect the post-acute care provider or service you can use. If you choose a post-acute care provider or service that is not covered under your insurance plan or not participating in Medicare, you may be responsible for the cost of care. If you are enrolled in a managed-care organization, you should check whether your chosen provider is covered by your insurance plan before you pick them.
