Diabetes Management

Our experts offer team-managed care of diabetes in adolescents and adults with an emphasis on management and education.

Diabetes Management Focused on Your Health and Well-Being

Diabetes is a long-lasting condition that affects your body’s ability to turn food into energy. Having diabetes puts you at risk for serious health complications, including heart disease, blindness, kidney failure, stroke and loss of toes, feet or legs.

There isn’t a cure for diabetes, but our specialists are experts at managing it so you can enjoy better health and well-being. Our endocrinologists, nurses, pharmacists and educators work together to develop a personalized diabetes management program for you.

Types of Diabetes We Treat

We provide care for adolescents and adults with one of the following types of diabetes:

  • Type 1 diabetes: This occurs when the body doesn’t make enough insulin. Insulin is a hormone that moves glucose (sugar) from your food into your bloodstream, where it’s used for energy. People can develop Type 1 diabetes at any age.
  • Type 2 diabetes: This happens when your body can’t use insulin properly. People have a higher risk of Type 2 diabetes if they are overweight, physically inactive, over age 45, have a family history of diabetes or are members of a high-risk ethnic group.
  • Gestational diabetes: This sometimes develops during pregnancy when the body can’t produce enough insulin to meet the extra needs of pregnancy. It is treatable and usually goes away after childbirth.

We also treat patients with a condition called prediabetes. People with prediabetes have higher than normal blood sugar levels. They may develop Type 2 diabetes without medical help.

If you have prediabetes, we work with you on lifestyle changes, such as exercising more and adding healthier foods to your diet. These can often prevent or delay you from developing Type 2 diabetes.

Diabetes Management with a Team Approach

Diabetes management begins with a comprehensive physical examination and health history. Eye exams are included since diabetes can damage your eye’s retina.

Our diabetes doctors and nurse practitioners will identify any other health risks associated with diabetes, such as high blood pressure, obesity, high cholesterol, and neuropathy, an often painful condition that affects the nerves in your hands and feet.

Then, our pharmacists and diabetes educators help recommend a plan for managing your diabetes and other conditions, such as high blood pressure and cholesterol.

Your multidisciplinary team also includes specialists in heart health, foot care and eye care, if needed. If you are pregnant or planning to conceive, your team will work with your obstetrician to monitor your care during and after pregnancy.

Your team will monitor your condition and care frequently. When you are in our diabetes management program, you can expect to hear from your diabetes management team often to check in and ensure you stay healthy.
